A non-surgical solution to support pelvic organ prolapse
A vaginal pessary is a soft, medical-grade silicone device placed inside the vagina to help support the pelvic organs. It acts like a scaffold, reducing symptoms like vaginal pressure, heaviness, or bulging. Pessaries come in a range of shapes and sizes and are fitted to suit your anatomy, comfort, and lifestyle.
Pessaries are one of several options available to help manage pelvic organ prolapse. For more information about conservative treatment options, see our Prolapse Physiotherapy page.
Who Is It For
Pessaries can be a helpful option for women with pelvic organ prolapse whose symptoms have not fully resolved with physiotherapy alone. They are commonly used:
-
When symptoms interfere with daily life or exercise
-
While waiting for surgical consultation or review
-
As an alternative to surgery
-
As part of postpartum care
-
To provide support during certain activities like lifting or sport
-
When added support is needed alongside pelvic floor muscle training
We recommend trialling conservative treatment first, such as pelvic floor retraining, bladder and bowel optimisation, and changes to lifting or toileting technique. If symptoms persist, a pessary can be added to provide additional relief and improve quality of life.

What to Expect at a Fitting
Your appointment will include a thorough assessment, including a vaginal examination to determine the most appropriate pessary type and size. We may trial different shapes or sizes and check the fit during activities like coughing, squatting, or walking to ensure comfort and function.
Finding the right pessary may take a few appointments. Our goal is to make the process respectful, collaborative, and free of pressure.
Ongoing Pessary Care
If you are managing your pessary independently, we will show you how to safely remove, clean, and reinsert it. Regular reviews are recommended every 3 to 12 months to make sure the pessary remains safe, comfortable, and effective.
Most pessaries last for several years but still require routine checks to assess for wear, tissue changes, or shifting of symptoms.
Safety and Medical Clearance
Before fitting a pessary, you will need a recent cervical screen and pelvic check from your GP. Pessaries are not appropriate in every case. Situations like active pelvic infection, vaginal dryness, some skin conditions, or silicone allergy may prevent fitting. Your physiotherapist will guide you through any concerns and help coordinate medical input if needed.
Are There Any Risks?
Pessaries are generally safe and well-tolerated, but like any medical device, they do carry some risks. The most common issues include: increased vaginal discharge or odour, pessary expulsion during certain activities, changes to vaginal tissue, difficulty with removal, new bladder or bowel symptoms, and vaginal infections and bleeding.
Severe complications are rare but may occur if pessaries are neglected and not checked regularly. To minimise risks, it’s essential to attend scheduled reviews and follow your physiotherapist’s advice on pessary care and maintenance.

FAQs – Pessaries for Prolapse
Do I need a referral to have a pessary fitted?
No referral is needed to see our Women's Health Physios, however before we can fit a pessary you will need a recent cervical screen and pelvic check from your GP. Feel free to get in touch if you have questions regarding this.

Can I exercise with a pessary?
Yes. In most cases, a well-fitted pessary can make it more comfortable to stay active. We’ll help ensure it feels secure during movement.
Does a pessary fitting hurt?
Most fittings are comfortable. Some women feel light pressure, but it should not be painful. We go slowly and stop at any point if needed.

What if a pessary isn’t right for me?
We’ll guide you through alternative options, including pelvic floor therapy, activity modifications, and specialist referrals if needed.
